Onze praktijk gebruikt nu Mitz, voor het online vastleggen van toestemmingen NL/ENG

In Mitz kiest u zelf welke zorgaanbieders u toestemming geeft om uw medische gegevens beschikbaar te stellen. Uw andere zorgaanbieders kunnen deze gegevens dan opvragen als zij deze nodig hebben voor uw behandeling.

Waarom is toestemming geven belangrijk?
Zorgaanbieders zoals de apotheek of huisartsenpost, hebben soms uw medische gegevens nodig van onze praktijk. Zij kunnen u dan beter helpen. Wij mogen uw gegevens alleen elektronisch uitwisselen met een andere zorgaanbieder als u dat goedvindt. Dit heet toestemming.

Vanaf nu maken wij voor deze toestemming gebruik van Mitz. Heeft ons al toestemming gegeven via bijvoorbeeld een formulier of Volgjezorg? Dan zetten wij deze toestemming in Mitz. U kunt deze toestemming zien, als u inlogt in MijnMitz.nl.

Hoe werkt Mitz?
Ga naar MijnMitz.nl Op deze website staat uitgelegd hoe Mitz werkt. U kunt inloggen met DigiD en direct u keuzes maken voor al uw zorgaanbieders. Deze worden veilig opgeslagen. Alleen uw keuzes staan in Mitz, uw medische gegevens niet. U kunt uw keuzes altijd aanpassen. Ze zijn direct geldig.

In Mitz, you choose which healthcare providers you give permission to access your medical records. Other healthcare providers can then retrieve this information if they need it for your treatment.

Why is giving consent important?
Healthcare providers, such as pharmacies or out-of-hours clinics, sometimes need access to your medical information from our practice. This helps them provide better care. We are only allowed to share your medical data electronically with other healthcare providers if you give your permission. This is called consent.

From now on, we will use Mitz to manage this consent. If you’ve already given permission through a form or via Volgjezorg, we will transfer your consent to Mitz. You can view this when you log in at MijnMitz.nl.

How does Mitz work?
Go to MijnMitz.nl This website explains how Mitz works. You can log in using your DigiD and immediately make your choices for all your healthcare providers. Your choices are stored securely. Only your consent preferences are saved in Mitz – not your medical data.
You can change your preferences at any time, and your changes will take effect immediately.
